Sorry for the inconvenience. Due to the number of decal and pass requests we have received, the online forms are having some technical problems. We are not receiving all decal and pass requests issued through the website. Please email your requests directly to LCMC@LakeCushmanMC.com or by fax 360-877-6713.
If you would include the following in your email or fax, it would be appreciated:
1. Your Name
2. A good phone number and email address
3. Any additional information for requests (i.e. would you like a hanging guest pass?, how would you like delivery of your passes, mail or pickup?, etc.)

Due to the minimal staff in the office, please allow 7 to 10 business days to complete that request. Decal and pass requests are being issued in the order they are received. We can not guarantee a delivery date at this time.
NOTE: The 2019 decals are good through June 1, 2020 this year due to the COVID-19 Crisis.
